How it works: Visit your primary care physician, eye doctor, or dentist for screenings, check-ups, vaccines, and more. Then, complete the brief Stay Well, Live Well 24-25 Survey to be entered into a monthly prize drawing. No test results or sensitive information will be required. Limit one entry per month.
