Consultation Requests
AT for Academics
Use this form to request a consultation for a student who may benefit from Academic AT. If the concern is related to handwriting, please be sure to consult with the student's occupational therapist (OT) prior to submitting the request.
AT for Communication (AAC)
Use this form to request consultation, trainings, support, or switch-activated devices from the Assistive Technology team. This form can be filled by any stakeholder requiring Assistive Technology support, including classroom teachers, paraprofessionals, SLPs, administrators, etc. If you are requesting a new AAC device or component or a significant change to the AAC system, please consult with your student's SLP prior to submitting a request.
